Themes on mileage claims

2 problems with the themes on the create mileage claim page.
1.) It does not select my default theme.
2.) The themes drop down is not formatted correct and contains some unknown themes http://justphp.d.pr/1lzES/1V00i0ju - it should be formatted like the invoice create page http://justphp.d.pr/10ovG/VtOC8vtq
