purchase requisition form?

Does anyone out there use a Purchase Requisition (PR) system? If so did you create your own form/template for staff to seek approval or did you find one online (if so where?). I know I could create a form myself but if someone else has done it (Clearbooks team?) then why re-invent the wheel? :)
