Add Notes Anywhere

I feel it would be of benefit to add notes anywhere in the system
2 useful areas for my business:
HR Function - adding notes to Employee file, so you can see what adjustments were made to each employee - eg. payrise, holiday adjustment, tax code change etc.
Bank Transactions - would be useful to be able to add notes to any transaction eg. 'awaiting a refunds so it stops us reconciling without having to keep notes separately in other spreadsheets etc.
