EC Sales List submission failed

Just went to submit my 3-monthly EC sales list, everything worked fine, I hit 'Submit' and it went to the record page with the latest submission marked 'Pending'. After a few minutes I refreshed the page, and it changed to 'Failed'. No hint as to what had gone wrong. I submitted it again: same result.

How do I start troubleshooting this?
